There’s lots of good reading that covers security accounts for SharePoint farms and the various accounts required. There are also standard recommendations such as changing the SQL Server port from 1433. But at the end of the day it comes down to the level of security vs. the management overheard to implement and maintain the security. At the end of the day each organization must have its own policy that covers internally accessible and externally accessible systems – both require different levels of rigor (paranoia).
